Request deletion of your account or personal data

To request deletion, email [email protected] from the address associated with your account.

What to include

State that you are requesting account or personal-data deletion and include the email address used to sign in. If you are associated with a facility, include its name or other non-sensitive identifier. Do not send passwords, access codes, patient information, prescription information, or protected health information.

Facility records

Deleting a personal account can affect access to facility-scoped operational records. We may need to verify the request and coordinate with the relevant facility administrator before processing it. We will explain any records that must be retained or that cannot be removed because of legal, security, or operational obligations.
